Read the guidance for exporting or moving horses and other equines to the EU and Northern Ireland before filling in this form.

You must complete this form if you both:

  • own or manage premises in England or Scotland
  • export or move equines from these premises (or someone else exports or moves equines from the premises)

Examples of premises include:

  • private stables
  • private yards
  • livery yards
  • equestrian event centres
  • showgrounds
  • racecourses

If you wish to export to the EU or Northern Ireland you must:

  • keep records of the species, categories, numbers of animals and identification of animals for at least 3 years
  • keep records of movement of animals into and out of the premises, births, and deaths, for at least 3 years
  • have at least one animal health visit from a vet every year, including an assessment of your animals for signs of diseases that are notifiable in the EU or Northern Ireland

When you have completed the form, email it to equine.registration@apha.gov.uk.

The Animal and Plant Health Agency (APHA) will give your premises a CPH number. Anyone who exports or moves equines from your premises to the EU or Northern Ireland should use this number.

If you’re in Wales, apply for a CPH through Rural Payments Wales (RPW).
