Important Information on Privacy and Security
When you start filling out the online application and need to stop, you should protect your information from being seen by non-authorized persons by clicking on the Log Out button. This will allow you to come back and finish the application later. Also, if you are filling out the online application in a public location, do not leave the computer while you are filling out the form because your information could be seen by non-authorized persons.
Important Information on Accessibility
The Self-Assessment and Online Application have been developed and tested using JAWS. If your browser does not support scripting, you may not be able to complete these forms online. As an alternative, you may access a computer at your local library, contact your local DCF Service Center or call toll free at 1-888-369-4777.
